Alcohol Self-Assessment Questionnaire

As a condition of your resolution for violating the UC San Diego Student Conduct Code, you may have been assigned to complete this questionnaire.

The Alcohol Self-Assessment Questionnaire will help you assess your own knowledge of campus policies on alcohol, as well as the potential outcomes for repeated alcohol policy violations.

The questionnaire will take approximately 10 minutes to complete, is free and can be completed online by following the link provided to you in the letter from your resolution meeting. You may also access the questionnaire by following the instructions below.

Instructions:

  1. Go to the Alcohol Self-Assessment Questionnaire.

  2. To complete this sanction, fill out the form as instructed. Be sure to review the documents on alcohol policies and sanctioning that are provided.
